2013 Lax-4-Life Chaperone Registration



Welcome to the 2013 Lax-4-Life Camp Chaperone Registration with the Minnesota Swarm!

This registration session will allow you to provide contact information and sign consents.  Please read through each page carefully to ensure that all information is provided accurately.

Application Process

All prospective campers must have their chaperons complete the online application process. Boys and girls ages 13-15 are elgible to attend the camp. NOTE: there is NO COST TO YOU to attend this camp. Applications will be processed on a first-come first-served basis until all camper spots are filled. Acceptance of a camper application will be verified upon receipt.

Once the camper is confirmed, the camp staff will verify the chaperons and representatives from each reservation. Then each camper and chaperon will receive a camp confirmation packet. Please read the camp confirmation packet carefully as it details the camp schedule, items to bring, as well as roommate & transportation information.

Before sending in this application, please make sure all signature areas are signed by the parent or legal guardian.  Please note: we have limited space for this camp so please make sure you are 100% confirmed to attend before sending in your registration.  If for some reason you need to cancel, please notify us no later than two weeks in advance of the start of the camp.

What to Bring to Camp?

Camper will be provided with a lacrosse stick, mouth guard and protective gear that they can wear during the duration of the camp.

Please make sure each camper brings:

1. Sneakers/tennis shoes
2. Comfortable athletic clothes (shorts and t-shirts)
   - the activities will take place outside on grass in the warm summer
3. Bed linens or a sleeping bag
4. Towels
5. Sunscreen and mosquito spray
6. Shower kit
7. Fan (if you have one)
8. Swim suit
9. Rain gear and a set of warm clothes
   - in case of bad weather

* We will also have a Camp Store that will have merchandise, healthy snacks and beverages for sale throughout the week.
 ** Video games and televisions are not allowed at camp. No acceptions.
